02 — Referral

When a joint assessment makes sense

Cardiometabolic prevention complements primary and specialist care; it does not replace it. Collaboration works well when roles and feedback channels are clearly defined. Typical reasons:

03 — Feedback

What comes back to you

After every visit, co-treating colleagues receive — with the patient’s consent — a structured report containing:

Findings

All findings in full.

Risk classification

Including the score used and the risk modifiers considered.

Action plan

Specific measures with target values, where appropriate a medication proposal for coordination.

Follow-up interval

A defined interval for follow-up.

Ongoing care naturally remains in your hands.
